What is phimosis and its manifestations
Phimosis - a narrowing of the prepuce (the fold of skin covering the glans penis), in which the head of the penis does not open or opens with difficulty and painfully. Phimosis may be temporary, reversible or intermittent due to recurrence of inflammation. There are four degrees of phimosis:
- Grade I: at rest the head of the penis exposed without problems, during erection, this requires little effort;
- Grade II: the erection
Erection - what hinders and what helps a man to be a man
head of the penis does not open at all, at rest - with or without hard effort;
- Grade III: the head of the penis does not open or opens only in the quiescent state of the penis and a considerable effort, but urination is not broken;
- Grade IV: difficulty urinating - urine released a thin stream or drops; head of the penis to reveal possible.
Why can occur
Phimosis can be congenital or acquired. The cause of congenital phimosis is most often a congenital lack of the connective tissue in the body, so it may be accompanied by other signs of changes in connective tissue (eg, heart valve malformations).
Acquired phimosis develops in the background of inflammatory diseases and injuries of the penis, which leads to cicatricial narrowing of the outer ring of the foreskin.
Age development of phimosis
At the age of two phimosis occurs in most children, and this is considered normal. But, nevertheless, children phimosis can give the same complications as phimosis adults, so the question of whether to treat it strictly solved individually. But if by 6-7 years the glans penis is not open, then surgery is required.
Phimosis in adults can be an extension of child phimosis, but can also occur during puberty
Puberty child - stages of a complex path
When there is an intensive development of the sexual organs and at some point there is a mismatch between the development of the penis and prepuce (foreskin is less than the head), the head of the penis does not open or opens with difficulty.
In adolescents and adults, men phimosis is prone to progression, which is associated with the emergence of erections and sexual life (these physiological acts cause microtrauma that end with scars, still more narrows the foreskin).
Complications
Both congenital and acquired phimosis can produce a number of complications. The most frequent complication is paraphimosis, balanoposthitis
Balanoposthitis - you need to find the reason
and the increment of the foreskin to the glans penis.
Paraphimosis - is infringing glans constricted foreskin. Such an infringement may occur during sexual intercourse or masturbation. This edema of the glans penis, it is blue, grows in size, it is sharply painful. Sometimes it is possible to straighten the head easily, but often this is not possible, then emergency surgery is needed.
Balanoposthitis - an inflammation of the glans penis and the foreskin that may occur due to lack of hygiene, which can be difficult. Accumulation of secretions in the area leads to the development of infections and inflammation.
The increment of the foreskin to the head can occur if expression of the degree of phimosis. At the same time attempts to open the head accompanied by sharp pain and bleeding.
What to do
Children up to two or three years without complications phimosis does not require treatment. Any conservative methods of treatment for phimosis adults ineffective. But children this treatment is carried out. Glans penis trying to get rid of the foreskin, gently pulling her hand. Sometimes, this is done using a metal probe, circular movements which the foreskin is separated from the head of the penis, after which the hole is cleaned of accumulated secretions, lubricate the head of vaseline oil and return the foreskin to its original position. In the future, the child's parents are advised to regularly (1-2 times a week) to expose the head and carry it sanitary processing.
If true phimosis circumcision is performed (the operation of the circular circumcision
Circumcision - religion against science
foreskin). In this operation, the foreskin is removed entirely or partially, whereby the head is completely exposed. Circumcision is shown for any degree of phimosis to prevent related complications. Contraindications for circumcision is also marked inflammation of the glans penis and foreskin - balanoposthitis.
When phimosis is often complicated by the need to urgently release the infringement of the glans penis. In this case, operation is conducted longitudinal incision of the foreskin. After the inflammation of the glans will be held, to be circumcised.
Prevention of phimosis
Prevention of phimosis is in compliance with the hyenas. The boys need every day to open the glans penis, and wash it thoroughly. If the head is opened with difficulty, it is necessary as soon as possible (before the appearance of complications) address to the urologist.
